Build a Registration or Survey Form
Section titled “Build a Registration or Survey Form”Collect exactly the information you need from attendees by building a multi-step form with conditional questions, optional payment collection, and a live view of the responses as they arrive.
Before you start
Section titled “Before you start”- You need the Organizer role, or Co-organizer on the event you are building for. Both roles can create, edit, publish, and archive forms and can export responses.
- Have the event or activity created first, so the form can be attached to it and its responses land alongside your guest records.
- If you plan to collect money through the form, make sure your event’s payment details are already set up. Without that, the payment question can be built but cannot take real payments.
Step by step
Section titled “Step by step”-
Open your forms and decide what to build. The forms list shows everything you have created for the event, with each form’s status and response count. Search by name, sort by newest, name, status, or most responses, and filter by form type or state to find something you can reuse. Publish, archive or delete a form from the form itself. To start something new, create a form from here.

Start from a template or from scratch. The template gallery offers ready-made starting points grouped into Events, Surveys, and General, each with an estimated fill-in time: Event Registration, RSVP, Workshop Registration, Speaker Application, Volunteer Sign-up, Feedback Survey, and Contact Form. Picking one drops its questions straight into the builder, already marked as required or optional, so you edit rather than build.

Shape the questions in the builder. Add and reorder questions, set which ones are required, and choose the right answer type: short and long text, email, phone, number, star rating, date, date and time, time, dropdown, multiple choice, checkboxes, and file upload. Use Section header blocks to group related questions, display-only text and image blocks for instructions or artwork, and a Page break wherever you want the form to continue on a new page. Attach conditional rules to a question so it only appears when an earlier answer calls for it, and add a payment question when registration requires a fee.

Check it the way a respondent will see it. The preview walks through your form page by page, with a progress indicator on multi-page forms, so you can confirm the wording, the order, and that your page breaks fall in sensible places. Questions you have kept for reporting only do not appear here, which is the point: the preview mirrors the real thing.

Publish and share. Back in the list, publish the form and copy or share its link. Anything you change afterwards applies to new responses.

Watch responses come in. The response dashboard shows one row per submission with a column per question. Narrow it down by filtering individual columns or by choosing a date range, and clear every filter at once if the results dry up. Uploaded files and images open directly from the table. Numbers, dates, ratings, and multi-select answers are each formatted for reading rather than shown raw.

Review performance. The analytics view summarises total views, total starts, total submissions, and completion rate, plus start rate, average completion time, and trends over time. Refresh it whenever you want current numbers.
Troubleshooting
Section titled “Troubleshooting”A field type is not doing what I expected. Match the type to the answer, not the question: Number and Rating store values you can average, while the same answer typed into a text field cannot be summarised. Section header, Page break, and display-only blocks are layout, not questions, so they never collect an answer and stay blank in the response table.
Conditional questions are not appearing. A rule only fires on an answer the respondent has already given, so the controlling question must sit earlier in the form, and on an earlier page if a page break separates them. Preview the form and answer it as a respondent to confirm each branch opens.
Payment is not being taken. Confirm the event’s payment setup is complete and the form is published, not still a draft. A payment question on an unpublished form collects nothing.
Export is missing rows. Exports follow what is on screen: active column filters and the selected date range both apply. Clear all filters first if you want the complete set, then choose your export format and wait for the confirmation before closing the view.
A question was deleted but old answers still show. That is intentional. Answers recorded before a question was removed are kept so historical submissions stay complete.
